The Miraculous Medal originated in 1830: 

One night, near midnight,  St. Catherine Labouré awoke by a voice whispering, “Sister, sister, sister.” Led by mysterious events to the chapel, the Blessed Mother appeared to St. Catherine Labouré and seated herself near the altar. As St. Catherine knelt at her feet, she rested her hands in Mary's lap, and looked into her eyes while she shared a heart-to-heart conversation. This night would later be recollected by St. Catherine as " the sweetest moment of my life".

Four months later, which would have been November of 1830, Mary appeared again to St. Catherine at the Rue de Bac Chapel. It was at this time that Our Blessed Mother revealed to St. Catherine her request for the production and distribution of this medal after sharing with her an apparition of exactly the imagery and inscription she desired.
